Course Objectives: To provide holistic care to the patient and family who need care at home, to assess the patient's health status in the home care process, to plan the nursing interventions, to apply and to evaluate the results of care, to make research to improve home care services, to ensure coordination with the hospital and the patient's own environment, to gain proficiency in communication with the health care team

Learning Outputs

The students who have succeeded in this course;
1• Define roles and responsibilities of home-care nurse

2 • Comprehend the definition of home care and its development process

3 • Obtain knowledge of home care services in Turkey and in the world

4 • Evaluate the process for getting care at home from the begining the patient being discharged, plan and apply appropriate nursing interventions

5 • Help the patient’s family to manage the process of care plan of the patient that gets home health care service

6 • Obtain knowledge of technology and equipment related to home health care service and gain proficiency in usage

7 • Coordinate and Organise the home health care service

8 • Obtain knowledge of the Home Health Care Services in Health Care Legislation and Discuss the importance of

9 • Work in coordination with the health team in home health care services and Gain proficiency in communication effectively

10 • Provide health education to the patient and the family that need care at home

11 • Follow research in the field of home care

Definition of home care, its benefits, home care services in the world and in our country, chronic illnesses and home care, home care practices and the role of nurse
